SUBJECT: MEDICARE FRAUD ALERT- STOLEN MEDICARE CARD

PRIMARY INTERESTS: BUSINESS OFFICE MANAGERS

The Health Care Financing Administration has informed our office that the Medicare card for the following individual has been lost or stolen in the Chattanooga, TN area.

Beneficiary Name: (Refer to hardcopy notice issued to provider)

HIC #: (Refer to hardcopy notice issued to provider)

Indications are that another individual is using her card to receive Medicare services throughout the country.

Providers that submit more than one claim to Riverbend GBA under this Medicare HIC number will be asked to furnish a copy of photographic evidence that the real Kimberly Lawson is the recipient of the service before we will pay.
